چکیده

Introduction To understand the impact of respiratory mechanics during mechanical ventilation, it is helpful to partition between the lungs and the chest wall. Esophageal pressure (Pes) is used to calculate chest wall compliance. However, esophageal pressure is not always used in the clinical arena. The value of chest wall compliance has been proposed to be estimated using 4% of the predicted value of vital capacity (VC) [1]. Objectives This study compared the difference between the predicted and the measured value of chest wall compliance in patients with ARDS.

Maneuver-free determination of compliance and resistance in ventilated ARDS patients.

At present, most methods of lung mechanics analysis do not take nonlinearities of compliance and resistance into account. Nevertheless, nonlinearity of compliance is an inherent property of the respiratory system in ARDS and nonlinearity of resistance is an inherent property of the endotracheal tube. Herein we describe a computer-assisted multipoint method (LOOP) for breath-by-breath calculatio...

Should PEEP Titration Be Based on Chest Mechanics in Patients With ARDS?

Functional residual capacity (FRC) is essentially the alveolar volume and a determinant of both oxygenation and respiratory system compliance (CRS). ARDS decreases FRC, and sufficient PEEP restores FRC; thus, assessments of PEEP by its impact on oxygenation and CRS are intimately linked. PEEP also can ameliorate or aggravate ventilator-induced lung injury. Therefore, it can be argued that PEEP ...

Targeting transpulmonary pressure to prevent ventilator induced lung injury.

Acute respiratory distress syndrome (ARDS) and ventilator induced lung injury (VILI) continue to challenge clinicians who care for the critically ill. Current research in ARDS has focused on ventilator strategies to improve the outcome for these patients. In this review, we emphasize the limitations of managing ventilators based on airway pressures alone. Specifically, basic pulmonary mechanics...

The complexity of interpreting plateau pressure in ARDS.

In the early years of lung-protective ventilation, it was thought that limiting plateau pressure (Pplat), a signifier of alveolar stress, was important for improving survival in patients with ARDS. However, there were no clinical data and no agreement on the limit. A 1993 consensus conference recommended that Pplat be limited to 35 cm H2O. This was based on the assumption that, despite heteroge...

Dosimetric comparison of TomoDirect and TomoHelical plans in post-mastectomy chest wall radiation therapy

Background: The purpose of this study was to evaluate the characteristics of TomoDirect (TD) plans compared to conventional TomoHelical (TH) plans in chest wall irradiation in patients with breast cancer. Materials and Methods: TD and TH plans for only chest wall were retrospectively created for 30 patients previously treated with TH technique in our clinic. The beam angles were arranged to cov...
